Social Work Supervisor (LCSW) - PACE

Schedule: Monday-Friday, 8:00am-5:00pm

The Social Work Supervisor leads and supervises the social work team by:

• Ensuring the delivery of high-quality psychosocial services that support older adults in remaining safely in their home and community.
• Maintain oversight of social documentation, ensuring compliance with policies and regulations.
• Ensure Social Workers engage meaningfully in IDT care planning and evaluations.
• Carry a caseload of participants by conducting home visits, completing assessments, and providing clinical care as needed.
• Provide crisis management and guidance for complex cases .

What is PACE?

PACE or Program of All-inclusive Care for the Elderly is a federal and state funded program that provides comprehensive, coordinated, and proactive whole-person care to help maintain and improve seniors' health so they can live safely in their homes. There are 188 PACE programs, serving more than 86,000 participants in 33 states and the District of Columbia.

The number of people aged 55+ is increasing in Grand Junction, CO and the demand for eldercare professionals is on the rise.

You Bring:

• Master’s degree in social work from an accredited social work program. LCSW required
• Employees must maintain registration or licensure thought their employment with HopeWest.
• 3+ years experience as a accredited social worker (LCSW)
• 1+ years’ experience in a leadership or supervisory role.
• Valid Colorado driver’s license, access to reliable transportation, and adequate automobile insurance maintained throughout employment.
• Able to work in a variety of environments.
• Knowledge base of community resources.
• Current CPR certification is maintained throughout employment.
• Excellent interpersonal and communication skills.
• Adequate computer literacy in MS Word, Excel, Access, Outlook.
• Effectively and respectfully communicate both verbally and in writing in a multitude of situations and settings, including highly emotional interactions.
• Effectively work with and build relationships with others.
• Personal commitment to working with diverse customers and colleagues.
