✨ Position Overview

As a Hospice RN, you will deliver patient-centered end-of-life care in a supportive and purpose-driven environment. Your role focuses on comfort, symptom management, family support, and ensuring the highest quality of care.

🩺 Key Responsibilities

• Provide comprehensive hospice nursing care and symptom management
• Conduct patient assessments and implement individualized care plans
• Educate and support patients and their families throughout the hospice journey
• Collaborate with interdisciplinary teams (physicians, social workers, chaplains, CNAs)
• Ensure accurate documentation and adherence to hospice standards

🎯 Qualifications

• Active RN license (eNLC accepted)
• Prior Hospice, Home Health, or Palliative Care experience preferred
• Strong communication, empathy, and critical-thinking skills
• Ability to work independently in a community/home setting
